STATEMENT OF NO CRIMINAL RECORD, MENTAL ILLNESS, OR SUBSTANCE ABUSE (Hand-write name after printing out registration)

I, _________________________________, hereby declare that I have no criminal history, mental illness, substance abuse history, or other conditions which by law prohibit me from being in possession of a firearm, driving a motor vehicle, or in any way prohibit me from partaking in this training. I further state that I am not currently under or waiting indictment, prosecution, under investigation, or other legal action for any criminal offenses.

STATEMENT OF TRAINING CONFIDENTIALITY (Hand-write name after printing out registration)

I understand that Global Assets courses are restricted to military, law enforcement, and government employees. I fully agree not to share or otherwise disseminate information or training to individuals who are not currently employed with US law enforcement and/ or government agencies, or who are not active members of the US Military without the express written consent of Global Assets Integrated.

Please complete all of the following to secure your seat:

1. Fill out this online form and click submit. You will be sent an email with a downloadable PDF. Please notify us if you do not receive an email within two (2) business days.
2. Print the PDF and fill out the areas that require your printed name, signature and date.
3. Email the filled out form by replying to the email you received (coursereg@gatgusa.com). You can also fax to 702-838-3466.
   • Make sure to include a copy of your department ID.
   • Include a copy of your previous FETT Explosive Breacher course certificate if applicable.

After your signed registration form is received you will receive an email confirming along with the Agency Training Approval letter, to be completed by a supervisor, and an invoice.

4. Complete and return the Agency Training Approval letter signed by a supervisor.
5. Submit payment or provide purchase order (PO) as applicable.

Terms and Conditions

1. A full tuition payment is required to secure your spot in one of our courses.
2. A full refund of tuition will be granted up to 14 days prior to the course’s start date. After this time, no refund will be granted, but tuition can be applied to another course.
3. Safety is a primary concern during all training courses. If, in the sole judgement of our instructors, a student’s ability to train safely falls below an acceptable level, our instructors reserve the right to remove that student from the course. Tuition will be refunded.
